Medical Record Information
As a United Digestive patient, there are several ways for you to access your medical information. The easiest way to request your records is through our online patient portal.
You can view and print your medical records through our patient portal, healow™. If you are a current patient with any UD partner practice, you should have received an email invitation to activate your new portal account. If you wish to have your invitation resent or you have not yet received credentials, please give us a call. We’ll be happy to send you an invitation so you can get started with these online services.
Request Medical Records
Although you can view and print your medical records online via our patient portal, all United Digestive partner practices require written authorization in order to release these records directly to you or a third party via fax, email or in paper format. This process is in place to protect your private health information.
You may also request your medical records by following these steps:
Make sure the patient or authorized requestor (if not the patient) signs the form
Submit your completed Medical Records Release form along with a copy of the requestor’s photo ID via one of the below methods:
