    • Complete the intake form sent via text or email 10-14 days before your procedure.
    • Confirm your appointment via the text prompt sent 5 days prior.

    • Arrive 1 hour before your appointment.
    • Review your confirmation email to verify appointment details.

    Thoroughly enjoy coming to see Dr. Fernandez and his office. The staff is efficient, thorough, and very friendly. Dr Fernandez listens and is very attentive, explaining his views in easy understanding ways. He leaves no stone unturned. Very, very good doctor.

    No Show / Cancellation Policy

    If you are unable to keep your appointment or procedure, please reschedule at least 48 hours in advance. Failure to do so may result in a $25 fee for a missed appointment and a $75 fee for a missed procedure.
